Abstract

Abstract A fluid operated retractor for use in surgery. The retractor has a portion which is expandable upon the introduction of fluid under pressure. The expandable portion is made of a material strong enough, and is inflated to enough pressure, to spread adjoining tissues within the body. The retractor is especially useful in fiber optic surgery because it can be inserted percutaneously through a small opening then expanded to a much larger dimension when in the desired location, to retract tissue from within. The retractor may be used to spread a joint such as a knee joint or a shoulder joint, or may be used to separate tissue planes generally, to improve visualization and create a working space for the surgeon.

Claims Analysis

Total Claims: 11
Claim 1
Independent Claim 110 words
A method of performing carpal tunnel surgery, comprising the steps of: inserting a retractor to approximately the location where the carpal tunnel surgery is to be performed, the retractor including a stalk having proximal and distal end portions and an expansible bladder connected with the stalk at or near the distal end portion of the retractor stalk for expanding tissue radially outwardly; expanding the retractor by introducing fluid under pressure to the expansible portion of the retractor to move body tissue radially outwardly at approximately the location where the carpal tunnel surgery is to be performed to facilitate performance of the carpal tunnel surgery; and performing the carpal tunnel surgery.
Claim 2
Claim 3
Dependent Claim 42 words
A method as set forth in claim 1 wherein said step of expanding the retractor to facilitate performance of the carpal tunnel surgery includes separating tissue such as tendons from the median nerve by introducing the expansible portion of the retractor therebetween.
Claim 4
Dependent Claim 68 words
A method as set forth in claim 1 wherein the expansible portion of the retractor is toroidal in shape and has a central instrument passage extending therethrough, and wherein said step of performing the carpal tunnel surgery includes inserting a surgical instrument for performing the carpal tunnel surgery through the central instrument passage in the retractor to the location where the carpal tunnel surgery is to be performed.
Claim 5
Dependent Claim 26 words
A method as set forth in claim 1 wherein said step of inserting a retractor comprises inserting a retractor through a small opening in the skin.
Claim 6
Dependent Claim 75 words
A method as set forth in claim 4 wherein said step of inserting a retractor through a small opening in the skin includes inserting a retractor having an unexpanded dimension and an expanded dimension which is greater than the unexpanded dimension, the expanded dimension being greater than the size of the opening in the skin, and wherein said step of expanding the retractor includes expanding the retractor from the unexpanded dimension to the expanded dimension.
Claim 7
Dependent Claim 24 words
A method as set forth in claim 1 wherein said step of inserting a retractor includes inserting a retractor having a rigid plastic stalk.
Claim 8
Dependent Claim 27 words
A method as set forth in claim 1 wherein said step of inserting a retractor includes inserting a retractor having an at least partially flexible plastic stalk.
Claim 9
Dependent Claim 30 words
A method as set forth in claim 1 wherein said step of inserting a retractor includes inserting a retractor having an expansible bladder made of a fiber reinforced composite material.
Claim 10
Dependent Claim 32 words
A method as set forth in claim 1 wherein said step of inserting a retractor includes inserting a retractor having an expansible bladder which expands asymmetrically around the stalk of the retractor.
Claim 11
Dependent Claim 31 words
A method as set forth in claim 1 wherein said step of inserting a retractor includes inserting a retractor having regulator means for controlling the pressure of fluid within the bladder.
