Abstract

Abstract A laser microsurgical method and apparatus are disclosed for safely treating occluded body lumens with laser energy. The disclosed method involves the use of pulsed laser energy from an argon-ion laser to destroy the tissue of luminal occlusions, such as atheromatous plaque and the like, with substantially no thermal necrosis of the surrounding tissue. A flexible catheter and associated laser microsurgical system is also disclosed which provides for aiming of the fibers to transmit the laser beam at the distal end of the catheter and rotational positioning of the catheter to permit coverage of virtually the entire cross-sectional area of the lumen. A lens system at the distal end of each laser fiber minimizes beam divergence and directs the beam toward the central axis of the catheter. A further embodiment of the catheter of the invention provides an array of laser fibers which are optically scanned with laser energy to cover the cross-sectional area of the lumen. The system is designed to minimize the possibility of damage to or perforation of the surrounding tissue of the lumen.

Claims Analysis

Total Claims: 8
Claim 1
Independent Claim 109 words
A method of performing laser angioplasty by effecting damage to an obstruction in the vascular system of a human comprising: generating a non-continuous wave, pulsed laser beam having a predetermined pulse duration, pulse repetition rate, pulse energy and duty cycle selected to operate substantially at the threshold of the thermal relaxation time of the irradiated volume of the obstruction such that damage is effected to the obstruction with substantially no thermal necrosis of the surrounding tissue of the vascular system; inserting a flexible optical fiber into the vascular system of a human; and transmitting said laser beam through said flexible optical fiber to the obstruction in the vascular system.
Claim 2
Claim 3
Independent Claim 179 words
A method of performing intraluminal laser surgery in a human body having a tubular lumen occluded or partly occluded by an obstruction comprising the steps of: providing at least one flexible optical fiber having a diameter smaller than the diameter of the tubular lumen, said fiber having proximal and distal ends; inserting said flexible optical fiber into said tubular lumen such that the distal end of the fiber is positioned adjacent the obstruction in the lumen and the proximal end of the fiber is out of the lumen; generating a non-continuous wave, pulsed laser beam having a pulse duration, pulse energy and pulse repetition rate; irradiating the proximal end of the fiber with said pulsed laser beam to thereby transmit the pulsed laser beam from the proximal end of the fiber to the distal end of the fiber; and controlling the pulse duration, pulse energy and pulse repetition rate of the pulsed laser beam so that the pulsed laser beam impinges upon and vaporizes the material of the obstruction with substantially no thermal necrosis of the surrounding lumen tissue.
Claim 4
Claim 5
Dependent Claim 34 words
The method of claim 2, including step of inserting a flexible tubular duct into said tubular lumen with said flexible optical fiber for introducing fluids into the lumen or exhausting fluids from the lumen.
Claim 6
Dependent Claim 20 words
The method of claim 2, including the step of impinging the laser beam selectively on different areas of the obstruction.
