Abstract

An instrument, method, and two-part plastic fastener for surgical stapling of tissue, the method comprising: A) placing the tissue between: 1) a retainer (26A) of absorbable material and located at the distal end of the tissues and having holes (174) that extend longitudinally through that material; 2) an elongated rigid piercing member (30A) which is located proximal to tissue, extends longitudinally, and has spikes (143A) on its distal end for piercing tissue; and 3) a tether (303) of absorbable material that is also located near tissue and has distal prongs (164) that upon distal movement of the tether can a) move distally through the openings pierced in the tissue by the piercing tips (143A) and b) then moving distally into the retainer hole (174) to hold the retainer and the attachment device together; B) then moving the piercing member (30A) distally so that its distal end pierces tissue; and C) then moving the retainer (30B) distally so that its prongs move through the drilled openings in the tissues and into the retaining holes (174) to hold the retainer and the tie together. The retainer holes (174) are designed to snap onto enlarged ends (164) on the fastener (30B) and thereby lock the two members (26A and 30B) together.
