Surgical instrument for suturing hollow organs
Abstract
Abstract 1,241,577. Suturing instrument. VSESOJUZNY NAUCHNO-ISSLEDOVATELSKY INSTITUT KHIRURGICHESKOI APPARATURY I INSTUMENTOV. 22 Aug., 1968, No. 40373/68. Heading A5R. A suturing instrument comprises a hollow body 1 containing a movable tubular rod 6, a movable handle 2, a staple ejector 9, a split ring washer 10 loosely set on a puncher 7 which is threaded onto the front end of the rod 6, a reciprocating stem 17 and a detachable head 19. As shown, the head 19 is screwed to the pointed end 18 of the stem and located by a guide key 25, and comprises a cylindrical part 22 and a connected tapered portion 21 which can rotate independently about its axis, portion 22 having a cylindrical recess 23 to accommodate a projection 11 on the puncher. The instrument is operated by removing head 19, locking the handle 2 with lock 15, withdrawing the stem 17 into the body 1 where it is held by swivel retainer 35 engaging in groove 33 and introducing the instrument perorally into the patient. The stem 17 is then moved all the way forwards and the pointed end 18 pierces the esophagus. The stem is then free to rotate about its own axis by virtue of the groove 34 engaging with a support washer (not shown) in the back of the body 1, beneath nut 31, thus permitting section 21 of the head to be fixed to the stem 17 by rotating the stem by means of its tailpiece 20. The stem is brought back close to the body 1, by tailpiece 20, with the lower tissue attached to it, the retainer 35 engages the slant groove 32 and is rotated to set the gap. The two tissues are thus brought together between the front of the puncher 11 and the recess 23 of the head 19. Lock 15 is released and handles 2 and 3 are brought together causing staples to be driven through the tissues and bent against depressions 24 in the back of head 19, and an excision to be produced between edge 37 of the recess 23 and bevelled edge 36 of the puncher 7.
