Claims Analysis

Total Claims: 13
Claim 1
Independent Claim 240 words
A surgical device (16) for performing a transmyocardial revascularization procedure on a patient's heart, said device comprising: a handle portion (20); a tubular neck portion (22) connected to said handle portion (20); a removable head member (24) on the distal end of said neck portion (22) having a through bore (64); piercing means (25) retained within said head member (24) and extending in use outwardly through said bore (64) for making an initial opening in the epicardium membrane of the heart; characterised in that said head member (24) has an end face (27) in which said through bore (64) opens, which end face is adapted to engage the outer surface of a heart being treated; and in that the device further comprises an optical fiber means (26) having a proximal end connectable to a source of laser energy; said fiber means (26) extending through said handle portion (20) and into said neck portion (22) with its distal end within said head member (24); control means (30,32) on said handle portion (20) for engaging said fiber means (26) for moving it axially therein and thereby moving its distal end through said bore (64) and beyond said head member (24); whereby said initial opening made by said piercing means (25) serves to facilitate passage therethrough of said fiber means (26) so that laser energy from its distal end can be used to form a channel in the myocardium of the patient's heart.
Claim 2
Dependent Claim 26 words
The surgical device of claim 1 including means (37) connected to said handle portion (2) for creating a suction at an opening at said head member.
Claim 3
Dependent Claim 47 words
The surgical device as described in claim 2 wherein said means for creating a suction includes a tubular means (36) extending from a vacuum source (37) and connected to said handle portion (20) for communicating with said head membe (24) to cause the suction at said epicardium.
Claim 4
Dependent Claim 71 words
The surgical device according to any one of the receding claims wherein said head member (24) comprises an annular member having a flange portion (86) with a transverse face which provides said end face (27) for engaging the epicardium of the patient's heart, a body portion (62) with a central bore (64), and an annular seat projection (68) in said bore (64) providing a portion of said bore of reduced diameter.
Claim 5
Dependent Claim 62 words
The surgical device according to claim 4 wherein said piercing means comprises: a tubular piercing member (25) retained in said body portion (62) of said head member (24), said member (25), extending through said opening and having a tapered tip portion (58), said piercing member having a bore that is large enough to allow slidable movement of said fiber means (26) therein.
Claim 6
Dependent Claim 25 words
The surgical device according to claim 5 wherein said head member (24a) includes an axially movable safety shield (72) extending around said piercing member (25).
Claim 7
Dependent Claim 35 words
The surgical device according to claim 4 including a penetration force cushioning means (70) in said central bore (64) of said head member (24) which is axially aligned and engaged with said piercing member (25).
Claim 8
Dependent Claim 29 words
The surgical device according to claim 7 wherein said penetration force cushioning means comprises a coil spring (70) in said head member engaging and aligned with said piercing member.
Claim 9
Dependent Claim 25 words
The surgical device according to claim 5 wherein said tapered tip portion extends approximately 0.20 inches (5mm) beyond said transverse face of said head member.
Claim 10
Dependent Claim 42 words
The surgical device according to claim 4 wherein said tubular piercing member (25) has an enlarged annular head member (60) having a diameter greater than said reduced diameter bore portion but less than said central bore away from said seat projection (68).
Claim 11
Dependent Claim 35 words
The surgical device according to claim 5 wherein said head member (24) is threadedly attached to said tubular neck portion (22) so that it can be removed to facilitate replacement of said piercing member (25).
Claim 12
Dependent Claim 41 words
The surgical device according to any one of the preceding claims including means (56, 57) for adjusting the rotational orientation of said neck portion (22) and thus the position of said head member (24) relative to said control means (30, 32).
Claim 13
Dependent Claim 42 words
The surgical device according to claim 12 wherein said adjusting means comprises a jam nut (56) on said neck portion (22) which is attached to said handle portion (20) and a projection (57) extending from said jam nut to facilitate manual turning.
